Phlebotomy Admission Forms

After completion of the phlebotomy program requirements, students are eligible for admission into the program and will need to complete the following forms:

1. High School and college (if applicable) transcripts Contact the last high school you have attended and any colleges, etc. you have attended and request that an official transcript or your grades be sent directly to the address shown on the right
2. Three References (using Reference Form)
3. Proof of current immunizations or immunity
4. Criminal Background Check Form
5. Phlebotomy Course Application Form with copy of license
6. Include a personal statement on the back of application, write or type a brief account of the following:
   
   • Your activities now or since you last attended school. The things you have accomplished that have given you the most satisfaction. What are your perceptions of the duties of a phlebotomist?
   • Why you think you would make a good phlebotomist?
