Transcripts

To have an official transcript of Southwestern Michigan College grades forwarded to another educational institution or agency, complete the Official College Transcript Request form and forward it to the Records and Registration Office. Official transcripts are released only after all financial obligations to the College have been fulfilled. There is no fee assessed for either an official or unofficial transcript request. No transcript is official unless it carries the college seal.

You may request a transcript via any of the following methods:

1. Send a letter requesting a transcript with a completed and signed transcript request form to the contact address shown on the right.
2. Complete the Official College Transcript Request form and fax a request to the Records Office at 269 782-1331.
3. Fill out the Official College Transcript Request form in the Records Office located in the College Services Building at the Dowagiac campus or at the main office located at the Niles Area Campus.

Whichever method you choose, you must include the following information in your communication:

• Name
• Maiden Name (or any other name you may have used while in attendance)
• Address
• Social security number and/or Student ID
• Telephone number
• Name and address of the institution where you would like the transcript sent
• Dates of SMC attendance
• Your written signature
